By being consistent, transparent and open-minded, we have created a culture that allows us to retain and build team members, patients, and community affiliates.  Every relationship is an opportunity for us to learn how best to continue this core value, even if it means that we don't start a relationship we won't be able to maintain.

Meet The Team

Co-Owner

Carrie Irvin, PA-C

Carrie is born and raised in New Mexico and has been in healthcare since 2000. Her undergraduate training and research in Nutrition and Exercise Science kindled her interest to be more involved and have a greater impact with her patients’ outcome. She went on to University of St. Francis to obtain her Masters of Science in Physician Assistant studies.

Since 2004 Carrie has cared for patients in various settings, including urgent care, long term acute care hospitals, acute rehabilitation hospitals, skilled nursing facilities, and substance abuse recovery programs. In 2011, she co-founded Rehabilitation Partners with her business partner, Mark Gould, PA-C. She is currently taking a more active role in practice management. Away from the office she enjoys spending time with her husband and three sons as well as many outdoor activities including hiking, cycling, and swimming. Carrie loves New Mexico’s great weather, beautiful scenery, and diverse culture.

Mark Gould, PA-C

Hello, I'm Mark Gould, a 55-year-old healthcare professional who has been a proud resident of Albuquerque since 1974. I graduated from Sandia High School in 1986. My commitment to healthcare was profoundly shaped by my mother’s battle with breast cancer, which she unfortunately lost in 1987. Her experience inspired me to dedicate my life to helping others. 

I began my career as a Registered Respiratory Therapist after training at TVI. I spent ten years working at UNMH while simultaneously completing a Bachelor's degree in Biology, with a minor in Chemistry, at UNM. Driven by a desire to further my medical expertise, I pursued and obtained my PA-C along with a Master’s degree from the University of St. Francis in 2005.

tation Partners LLC with my business partner, Carrie Irvin, PA-C. Our practice has flourished into one of the few independent, locally-owned practices in the Albuquerque area. We provide comprehensive internal medicine consultations across various settings, including skilled nursing facilities, long-term care, substance use disorder centers, and behavioral health hospitals. Additionally, we offer house call services for patients unable to leave their homes. 

I am fortunate to be married to Christie, whose love and support are invaluable. Together, we take great pride in our children. Outside of my professional life, I enjoy traveling, staying active through workouts, and spending quality time with our playful puppy, Sully.

Physicians Assistant

Ben Lohr, PA-C

Ben Lohr obtained his undergraduate from University of New Mexico and Masters from University of St. Francis. He enjoys exercising, rock climbing, and spending time with his son. Ben is originally from Tennessee but has lived in Albuquerque since 2002.

Nurse Practitioner 

Debra Green, CNP

Debra began her career as an LPN in 1984. She continued on at Odessa College in Texas to gain her RN and associate's degree in nursing in 1985. Debra finished her BSN from the University of New Mexico in 2000. She continued on to University of St Francis for her MSN and became a certified nurse practitioner. Debra has been with Rehabilitation Partners since 2016.

She enjoys spending time with her eight grandsons and three granddaughters. Debra also likes to camp, hike and swim when the weather permits. She also loves to read in her free time Thrillers are her favorite.

Physician

Kathleen Johnson, MD

Dr. Kathleen S. Johnson joined Rehabilitation Partners, LLC in July 2019.   She completed her medical training as well as her psychiatric residency at the University of Iowa. Dr. Johnson has been a consulting psychiatrist for many years in Albuquerque and maintains a private practive in the city. She offers medical directorship support and psychiatric consultation at various locations served by Rehab Partners.  Dr. Johnson enjoys living in New Mexico and the many outdoor activities available.  

Physicians Assistant

Jacob Nash, PA-C

Jake is originally from the Chicagoland area and moved to Albuquerque with his wife about a decade ago. He was originally a medical librarian which spurred his interest in medicine. He attended the PA program at the University of St Francis and graduated in 2023. Jake joined Rehab Partners in 2023 and currently works with patients at Haven Behavioral Health Hospital and Clear Sky Rehabilitation Hospital. He enjoys hiking, meeting new people, and lazy Sunday mornings with a good book and plenty of black coffee.

Family Nurse Practitioner 

Moriah Mahoney, CNP

Moriah is a family nurse practitioner with over 20 years of experience treating patients of all ages, and in recent years specializing in the needs of adults with mental health and addiction related concerns. She is a certified Addiction RN and has been treating patients affected by the opioid epidemic since 2017. 

She has been treating patients with Rehabilitation Partners since 2021 in acute psychiatric and substance use treatment settings. 

When she is not treating patients she is spending time in the mountains outside of Albuquerque with her dog and family, or in the garden or on her bike!

Practice Management and Billing

Noelle Kriling

Noelle joined Rehab Partners in 2022 and specializes in practice management and billing services. She has a a degree in Health Information Management that she obtained in 1995 when HIM was in its infancy . Little did she know how this field would take off and expand so greatly in the business part of healthcare! Throughout her career,she has obtained credentials in coding, auditing, analysis , contact specialist , law, data management and business. She moved to NM from Texas 2020 and is enjoying living out west .

Medical Doctor

Dr. Ralph Hansen

Dr. Hansen received his undergraduate degree in microbiology from Oregon State University, and his medical degree from the Oregon Health and Science University.  His postgraduate training took place in the UCLA system, primarily at Cedars-Sinai Medical Center, where he completed a residency in internal medicine and a fellowship in infectious diseases.  He practiced medicine for many years in Los Angeles providing infectious disease consultations and primary office based internal medicine.  He developed a unique expertise in HIV/AIDS care, and was medical director of a dedicated HIV/AIDS ward which opened at Century City Hospital in 1986.  Dr. Hansen was active in teaching medical trainees as an Associate Professor of Clinical Medicine at the UCLA School of Medicine.  Additionally, he served as Chief of Staff of Century City Hospital, and was clinical chief of infectious diseases at both Century City Hospital and Cedars-Sinai Medical Center.

Having had enough of living in the Los Angeles megalopolis, he relocated to Albuquerque in 2009, where he worked for the New Mexico Department of Health serving as medical director for the Developmental Disabilities Support Division and the NW Public Health Region.  It was important work, but Dr. Hansen missed direct patient care and being "at the bedside."  He joined Rehabilitation Partners in October, 2015, where he practices old fashioned patient centered medicine, and has time to talk to families and coordinate care with facility nurses and therapists.  Dr. Hansen currently serves as medical director of Advanced Healthcare in Albuquerque.

Dr. Hansen and his wife, Pauline, have a blended family with four children and ten grandchildren.  They enjoying traveling to keep up with the kids and grandkids in Bakersfield, San Francisco, Seattle, and Virginia Beach.  In his spare time, Dr. Hansen enjoys photography, reading, music, and grilling on the BBQ.

Doctor of Nursing Practice

Decy Devere, DNP

Dr. Decy Devere is a Doctor of Nursing Practice and a Board-Certified Gerontological, Adult/Gerontology Primary Care, and Psychiatric Mental Health Nurse Practitioner. She completed a Doctor of Nursing Practice Degree at the University of Texas Medical Branch at Galveston and her third post-master's certification as a Psychiatric Mental Health Nurse Practitioner at the University of Cincinnati. She earned a Master of Science in Nursing, two post-graduate certifications as a Gerontological Nurse Practitioner from the University of Texas Health Science Center at San Antonio and as an Adult/Gerontological Primary Care Nurse Practitioner from the University of Massachusetts Boston and is Board Certified by American Nurses Credentialing Center. She earned a Medical Laboratory Technique degree from George Washington University, a double degree in Biology and Marine Science, and a Bachelor of Science in Nursing from the University of Hawaii at Hilo. 

Decy served our military in the United States Navy (USN). Enlisted: August 18, 1986 - Discharged: March 16, 1992,  Disabled Veteran, Operation Desert Storm. 

 She has extensive experience in the healthcare field since 1980, from being a paramedic to a registered nurse practitioner for our acute, psychiatric, and behavioral health, post-acute, palliative, hospice, and senior communities. She currently serves patients at Central Desert Behavioral Health, and has also joined Rehab Partners in order to provide psychiatric and substance recovery care for patients at Vanguard Behavioral Health.  

 In her free time, you may find her riding her Tera Trike Recumbent bicycle on the bike trails or jetting off to a remote Caribbean dive location for a little 'hydro-therapy' as she explores the exotic blue ocean ecosystem. While at home, she enjoys caring for numerous 'special needs' kitties, ones that were deemed 'unadoptable' by shelter criteria. She has been actively involved in volunteering and giving time to ABODE Contemplative Care for the Dying - An interfaith organization created to provide a home for people in need near the end of life, as well as Wildlife Rescue and Rehabilitation, created to rescue, rehabilitate, provide sanctuary and individualized care to native species and other animals in need.

Nurse Practitioner

Michelle Barone, CNP

Michelle is originally from South Florida and has over 30 years of healthcare experience. She first began her medical career as a Paramedic in emergency services in the 1990’s. This triggered her interest in nursing. She graduated from Florida International University with her BSN in 1997 and began working as an emergency department nurse. 

She moved to St. Louis , Missouri and entered Maryville University to complete her Adult Nurse Practitioner program and graduated in 2010. She joined the Air Force Reserves while attending university and served as an Aeromedical Evacuation Flight Nurse and Major since 2008. 

She has worked in various settings as a Nurse Practitioner, including primary care, employee health, urgent care, long term care, and internal medicine. Additional roles included clinical team leader and preceptor. At present, her duties with Rehab Partners are to provide medical care for patients in a post acute rehab setting. She has been with Rehab Partners since 2017. 

Michelle moved to NM in 2012 after visiting family because she fell in love with the weather, mountains, and culture. She spends her free time mountain bike riding and racing, hiking, and skiing. She loves outdoor adventures and has a passion for animals. Michelle has two rescue dogs and a 100 pound Boxer!!"

Physicians Assistant

Pamela Snow, PA-C

Pamela was born and raised in rural Minnesota on a dairy farm, the oldest of 6 kids! 

She graduated from her nursing program in 1983 and worked as a RN until 1996. She then pursued both a Bachelor’s and Masters Degree in PA studies at The University of Nebraska and graduated 1999. She also completed multiple semesters through St. Johns College in Minnesota clinical pastoral education. She worked alongside different religious leaders taking care of patients in state run mental health settings. 

Pam has a wealth of experience in healthcare, working in various settings including oncology, critical care, psychiatry, and long term care. She is currently taking care of patients at Clear Sky, as well as South Valley Care Center. Having worked in the Albuquerque area for close to 30 years, she currently has some patients that she has cared for off and on for all of these years. 

When Pam is not working, she enjoys hiking in state parks in both Minnesota and New Mexico. She is also involved in her local church and volunteers extensively.

Brooke Kivitz, PA-C

Brooke has worked with Rehab Partners for a number of years, taking care of patients at Spanish Trails, Advanced Healthcare, and Vanguard Behavioral Health. She has a wealth of experience as a PA, working in various settings such as internal medicine, urgent care, pediatrics, palliative care, and vascular surgery. She graduated from the University of St. Francis in 2008.
